APDR is the UK SEND Code of Practice cycle for supporting a child's needs. You start with an observation (Assess), plan an intervention, do it, then review whether it helped. Each stage has its own notes.

How to

  • Start a new cycle with a short assessment note
  • Advance to Plan when you've decided what to try
  • Advance to Do when you've started the intervention
  • Close the cycle with a Review note describing what changed

Compliance and safeguarding

This is the statutory graduated approach from the SEND Code of Practice (2014, as amended). Schools use it to demonstrate to Ofsted that they are providing individualised support. EverywhereX records the cycle as a narrative scratchpad — it is not a replacement for the school's statutory record but it is a faithful capture of the same structure.
